WWE's television titles are hand-tooled metal plates finished in real gold plating, mounted on genuine leather. The gemstones you see are mostly cubic zirconia and colored synthetic stones, not diamonds (the famous exception: Ted DiBiase's Million Dollar Championship, which really did carry diamonds).

Two reasons. Cost: a championship-sized plate in solid gold would cost far more in raw metal than the finished belt is worth, before a single plate was tooled. Softness: pure gold bends and scratches with normal handling, a belt that gets held aloft, worn on shoulders and thrown over turnbuckles would be ruined in a month. Gold plating over a hard base metal gives the look of gold with the durability the metal underneath provides. That is why plating is the standard approach for a belt meant to be worn and handled.
